Care Coordination

At Senior Services, social workers, or “Care Coordinators,” help coordinate care and services to meet our clients’ needs. The goal is to provide assistance so that older people can remain at home. Sometimes seniors need services such as transportation to a doctor's appointment or someone to come to their home and help with housekeeping. Others may need help paying for prescriptions or could benefit from attending a support group. Senior Services has programs for these and many other needs. A Care Coordinator can set up services as needed.

In addition, a Care Coordinator is an expert in other community resources available for seniors. The Care Coordinator can recommend organizations to help with legal issues, food commodities, vision screening, medical equipment and many more. These resources are combined with Senior Services programs to help seniors live at home longer and maintain their quality of life.
